Direct Support Professional

Community Habilitation Specialist

Part-Time; Hourly, Non-Exempt

 

Immediate Supervisor:

Community Habilitation Supervisor

 

Position Requirements:

Minimum of one year experience working with the disabled population in any capacity; High School Diploma or Equivalent; Valid and clean driver’s license; successful completion of criminal background check

 

Position Responsibilities:

  • Provide Community Habilitation services to consumer(s) in their resident based upon a written plan
  • Maintain regular documentation of services during each visit by completing Individual response and daily notes.   At the end of each month, a monthly summary must be completed for assigned consumer
  • Provide transportation to and from areas within the community (with own vehicle), based upon the consumer’s individual goal, if required by the family.
  • Submit completed paperwork to supervisor by the third business day of following month in which services were provided
  • Attend consumer’s ISP review (if requested by the family) to discuss the progress of goals and the Community Habilitation plan
  • Maintain communication with direct supervisor on all issues as they relate to the consumer
  • Perform other duties as assigned

JOB COACH

JOB DESCRIPTION

 

REQUIREMENTS: The candidate for this position will possess a High School diploma and have some experience working with the disabled population or some job coach experience. A clean driver’s license is a must for this position.

 

JOB RESPONSIBILITITES:

-Find out job assignment for the day

-Find out clients assigned to you for the day and any important information regarding the clients

-Transport clients to and from work sites via agency van if necessary

-Provide direct supervision and orientation to clients while working

-Ensure all required job tasks are completed satisfactorily

-Functions as a role model for positive work behavior

-Communicate all client behaviors and issues to SEMP Supervisor and case manager

-Complete required ‘job site specific’ paperwork

-Attend weekly staff meetings as appropriate

-Maintain communication with SEMP Supervisor

-Communicate overall site performance to team

-Be aware of and implement documented client employment goals and objectives

-Follow and adhere to all agency and program specific policies and procedures

Bi-Lingual Medicaid Service Coordinator

35 Hour Workweek; Full-Time; Hourly; Monday-Friday 8am-4pm, with flexibility

Medicaid Service Supervisor

 

Position Requirements:

Bachelor’s degree in Health or Human Services with minimum of three years experience working with the disabled population or a Master’s degree in Health or Human Services; Ability to communicate fluently in Spanish; Successful completion of criminal background check; Strong verbal and written communication skills; Requires solid time management and organizational skills as well as robust computer skills.

 

Position Responsibilities:

  • Responsible for all aspects of case management, including advocacy, for a caseload of up to (not exceeding) 40 clients.
  • Attend and maintain documentation of professional development trainings to annually meet MSC guidelines.
  • Develop, implement, and maintain individual ISP’s as per MSC requirements.
  • As necessary, complete paperwork regarding withdrawal from or changing vendors.
  • Ensure individual client face-to-face visits are completed according to OPWDD regulations and time frames.
  • Ensure health & safety needs are met in client’s residence.
  • Maintain all necessary paperwork/documentation in case files.
  • Complete monthly case notes for all clients.
  • Provide all billing information to Supervisor according to OPWDD time frames.
  • Ensure SSI and Medicaid benefits are maintained.
  • Attend MSC program reviews, agenda meetings, & case reviews as necessary.
  • Perform other duties as assigned.

Direct Support Professional

35 Hour Workweek; Full-Time; Hourly, Mon, Wed, Fri -10a-6p, Tues and Thur-8a-4p

 

Immediate Supervisor:

Pre-Vocational Supervisor

 

Position Requirements:

Two (2) or more years of experience working with the disabled population; Knowledge of running consumer goals and computer skills a plus; High School Diploma or Equivalent required; A Successful completion of criminal background check; Excellent verbal, written and organizational skills required; Candidate must be a team player and have a willingness to take direction as appropriate;

 

Position Responsibilities:

  • Assist the Prevocational program with the responsibility of monitoring front door for morning bus arrivals and afternoon departures (i.e., Record consumer attendance, make phone calls as necessary to group homes/families regarding the whereabouts of a consumer that did not arrive to program)
    • Run set consumer goals while providing continued training and monitoring of active treatment by Production staff
  • Complete daily documentation of service provided, and include consumer response; two (2) goals are required for full-time participants, and one (1) goal for part-time participants; this includes documenting attendance sheets for each consumer.
  • At the end of each month, gather all goal documentation and write a monthly note for each consumer.
  • Conduct quality checks in all books to ensure the following are present; the last 3 ISP’s and prevocational plans, 12 months of dailies and monthlies, face sheet and LOC’s are current.
  • Provide coverage at lunch in the cafeteria
  • Provide bus coverage and supervision to consumers at dismissal
  • Three days per week, provide supervision and recreational activities in the After Hours program
  • Communicate any issues to Prevocational Supervisor as they arise
  • Maintain communication with Prevocational supervisor and case managers regarding consumer incidents, needs or behaviors observed during the day
  • Implement behavioral/training plans established by Treatment Team
  • Attend all required agency meetings
  • Perform other duties as assigned

Direct Support Professional Position Description

Day Habilitation

Full-Time; Non-Exempt; Hourly; Monday through Friday and weekends as necessary

 

 

Immediate Supervisor:

Day Hab Supervisor

 

Position Requirements:

High School Diploma or Equivalent with some experience (preferably 3+ years) working with the developmentally disabled population, Clean and valid NYS driver’s license required; Successful completion of criminal background check

 

Position Responsibilities:

    • Complete attendance list and program notes on a daily basis
    • Assist team in development and implementation of daily consumer curriculum
    • Assist consumers with the curriculum tasks as outlined
    • Provide direct supervision to and interact with the consumer in the Day Habilitation program
    • Maintain daily and monthly case notes
    • Participate in the Agency’s in-service training as requested and attend any other trainings that deal directly with the Day Habilitation program
    • Transport consumers to Agency outings as scheduled and adhere to all safety requirements
    • Supervise consumers during scheduled outings and assist with task building as outlined in daily curriculum
    • Maintain contact with Day Habilitation Supervisor at all times and bring items that require assistance to their attention in a timely fashion.
    • Communicate with family members, service coordinators, and case managers as required or requested by the Agency
    • Cross train in other programs
  • Perform other duties as assigned

Internship Program at Keon Center

 

On the recommendation of the Administrative Team, students will be placed in a program based on interest/education needs. Students will have the opportunity to gain hands on experience with the Intellectually/Developmentally Disabled and Dual Diagnosed population in a variety of areas such as Case Management, Pre-Vocation, Supported Employment, Day Habilitation as well as others.

 

Experience will include but is not limited to:

 

-Orientation to the Keon Center: policies, rules, funding, goals, etc.

-Orientation including ethics, regulatory training, person centered planning, etc

-Learn about eligibility for services

-Be exposed to Intake/Evaluation

-Complete 60 day evaluations as appropriate

-Opportunity to observe staff-run groups

-Opportunity to plan and run group of choice

-Attend various types of team meetings

-Opportunity to work 1:1 with I/DD and Dual Diagnosed individuals/cross train in various programs

-Become familiar with regulatory paperwork and program specific requirements

-All information learned regarding individuals receiving services will fall under confidentiality regulations and guidelines

-Staff at Keon will complete evaluations, progress reports from the college as requested

-All interested students must undergo background clearance that includes fingerprinting

Dennis Keon.

 

Dennis was one of our original consumers, and we had the pleasure of sharing his life for over 50 years.

 

Dennis' family were pioneers. They raised Dennis at home and he truly became an active part of the community. We are named after Dennis' father who was instrumental in giving us the brick & mortar of our first school.

 

While we have grown over the years from a school to an agency now serving disabled adults in a variety of programs, we have maintained the Keon name to honor the efforts put forth by Dennis' family in fostering the dream of community living and reaching goals that at the time seemed unreachable.

 

We are truly blessed for all that Dennis gave over the years. He will always be with us.
